The Exception Ledger is a record of every real deviation from a company's documented process, with how often each one occurs and who absorbs it today. It exists because the written procedure and the work as it is actually performed are rarely the same thing, and the gap is where the time goes. It cannot be assembled from documentation or a single interview — only from observing the work being done. SUPALABS produces one per engagement, for that process alone; the output is not reusable.
